Dear Patients/Parents:

It is with mixed emotions that I am announcing the closure of my practice, located at 8585 Sunset Drive, Suite 201, Miami, Florida effective April 30, 2018. It has been a great pleasure providing for your and your child’s health care needs over the years.

As of May 1, 2018 Dr. M. Edward Guthrie will be taking over my practice. I am pleased that you will have the opportunity to have him as your physician. Dr. Guthrie is an extremely well-trained graduate of the University of Alabama, Birmingham (UAB) College of Medicine where he also completed his ophthalmology residency. He completed his fellowship in pediatric ophthalmology and adult strabismus at Cedars-Sinai and Children’s Hospital of Los Angeles. He has been in practice in South Florida since 2001. I am glad to be leaving my patients in his capable hands. His office is located at:

Of course, you may seek medical care from another doctor if you like. If you choose to do so, I recommend looking for a new physician as soon as possible. A list of fellowship trained pediatric ophthalmologists can be found on the web site of the American Association of Pediatric Ophthalmology and Strabismus www.aapos.org or by emailing aapos@aao.org.

Your medical record is confidential and a copy can be transferred to another doctor or released to you or another person you designate only through your written permission. If you plan to continue with Dr. Guthrie no request is necessary as he will become the legal custodian of the records on May 1, 2018. If you choose to see a different physician, you will need to send an authorization form and request copies in writing.
